List the links you want connected to your site — social media profiles, booking platforms, payment pages, webinar links, etc. Your package determines the maximum number included.
Please read the following carefully. These terms protect both of us and define what a successful project looks like before we begin.
Revisions. All packages include revisions submitted within 20 calendar days of the delivery date. A "revision" is defined as a modification to existing content, layout, or design elements already included in the agreed scope. Revisions do not include new pages, new sections, or features not originally specified in this intake form. Requests submitted after the 20-day window will be quoted as a new project or hourly work.
Scope of Work. The scope of your project is defined by your selected package and the details submitted in this form. Any additions requested during the build process that exceed your package inclusions (additional pages, additional photos beyond package limits, new features) will be communicated and quoted before work begins on those additions.
Content Submission. You are responsible for providing all written content, photos, and brand assets within 3 business days of project kickoff. Delays in content submission will result in a corresponding delay to your delivery date. Bella Vita Wellness and Solutions is not responsible for delivery timeline extensions caused by late content.
Photos. The Seed includes up to 2 photos. The Bloom includes up to 8 photos. The Sanctuary includes up to 15 photos. Photos in excess of your package limit will be discussed during kickoff. All photos submitted must be owned by you or royalty-free. Bella Vita Wellness and Solutions is not liable for copyright issues arising from client-submitted media.
Payment. A non-refundable 50% deposit is required before any work will begin — no exceptions. Deposit funds must be fully cleared and confirmed in Bella Vita Wellness and Solutions' account before project kickoff. The remaining 50% balance is due in full upon project completion, prior to any site preview, file transfer, or launch authorization. Work will not be presented, transferred, or made live under any circumstances until the final balance has cleared in full. All payments are processed exclusively through a secure Stripe payment link. Accepted payment methods include credit card, debit card, Affirm (buy now, pay later), Apple Pay, and Google Pay. No payments are accepted outside of the Stripe payment link.
Ownership & Transfer. The completed website remains the exclusive property of Bella Vita Wellness and Solutions and is hosted within Bella Vita Wellness and Solutions' environment until full payment has been received and confirmed. Ownership, files, credentials, and all associated assets transfer to the client only upon receipt of final cleared payment. Bella Vita retains the right to display completed work in its portfolio unless otherwise requested in writing prior to project completion.
Chargebacks & Payment Disputes. By submitting this form and initiating payment, the client acknowledges and agrees that all deposits are non-refundable once work has commenced. In the event that a chargeback, payment reversal, or dispute is filed with any financial institution or payment processor, Bella Vita Wellness and Solutions reserves the right to immediately suspend all access to, and operation of, the produced website. The site will be taken offline and remain unavailable — including to the client and any end users — until the dispute is fully resolved and all outstanding balances are cleared. Bella Vita will submit complete documentation to the payment processor in defense of all disputes, including but not limited to: the signed project agreement, scope of work, proof of deposit, work-in-progress records, and all written correspondence. Clients who initiate chargebacks forfeit all active revision rights and project timeline guarantees for the duration of the dispute period. Bella Vita pursues all payment disputes to full resolution.
Communication. All project communication will be conducted via email or an agreed messaging platform. Response time from Bella Vita Wellness and Solutions is within 1 business day. Client responses are expected within 1.5 business days to maintain the delivery timeline.
Refunds. Due to the custom nature of this work, deposits are non-refundable once the project has begun. If you need to pause or cancel, please communicate immediately — we will work with you on a fair resolution.
